Our hospice program is currently looking for an RN Field Nurse/Case Manager to join our team. Because Sivitz Hospice is a small program, you will have the opportunity to provide individualized care for your patients.
The Hospice RN Case Manager is a member of the interdisciplinary group who works under the general direction of the Medical Director and/or the attending physician in the provision of skilled nursing assessment, planning and coordination of services in order to maximize the comfort and health of patients and families.
Required Education, Training and Experience for Hospice RN Case Manager:
Graduate of a school of nursing accredited by the National League of Nursing, Baccalaureate degree in nursing preferred.
Registered, certified or licensed as required by State Law on the profession.
Pennsylvania driver's license, current auto insurance and access to a car during working hours.
Hospice /Home Health experience with terminally ill patients is preferable.
Required Skills and Abilities for Hospice RN Case Manager:
Demonstrate flexibility, excellent assessment skills, ability to supervise others and the confidence to work independently.
Communicate effectively within a team.
Manage time efficiently.
Show concern for the provision of quality services.
Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.
